Structured Contact Form

I am currently using another app with this so I can collect necessary information from customers to one touch solve tickets with my team.

The idea is you can have a Help Category dropdown with a few options, then have other fields appear / disappear as needed.

This allows me to capture photos of damaged items, updated shipping addresses, photos of package contents when the customer believes there is missing items etc.

This also helps streamline tagging and sorting of common issues, in addition to tracking submissions of a specific issue type to diagnose and solve problems that are more common than others.

Gorgias has something similar, but I don’t think you can customize the form.

Even if the form is something that is very basic that applies to all ecommerce brands (without customization) I think it would be very helpful.

Adding this inside the help center would be very useful, as customers could be redirected to the form if they search for something that does not have a help center topic around it.
